How do I register with a GP in Fareham?
The NHS says you can register online by entering your postcode on the NHS "how to register with a GP surgery" page and choosing a practice in your area. The online form takes about 15 minutes per person and you would typically get confirmation within 5 days. You can also visit a surgery and ask for a paper form.
You do not need ID to register. You will be asked for your name, date of birth, address, an emergency contact and details of your current GP if you have one.
